§ 78-511. Forms.

(a)

Documents and papers necessary to the implementation and enforcement of this chapter shall be on such forms and in such manner as may be prepared and directed by the division manager of the business license division.

(b)

Such forms shall include the SIC (Standard Industrial Classification) of the business, its taxable gross revenues for the preceding 12 calendar months, the number of employees, and any such other information as may be required by the division manager of the business license division.
